Community Health Needs Assessment/Clinical Integration: Information and resources to identify community health and social service needs to better integrate prevention and education services with primary care and address access and availability.

Community Health Workers: Help individuals navigate the health care system, access insurance coverage and connect to appropriate resources.

MaineHealth Food Pantry/Food as Medicine: Access to food and personal supplies through provider referrals and community assistance. Hours: Monday & Wednesday: 10 am – 2 pm; Friday 11 am – 3 pm.

Matter of Balance: Program designed to reduce the fear of falling and increase activity levels among older adults.

Mobile Health Unit: Health screenings, education, resources and special programs.

Nutrition Education/SNAP-ED: Classes for children, adults and families on healthy eating, cooking on a limited budget, meal planning, reading nutrition labels, gardening education and interactive grocery tours.

Obesity/Let’s Go!: Connections for local schools and childcare sites to Let’s Go! And healthy nutrition. Physical activity opportunities and resources for youth and adults.

Resource Connections: Basic needs assistance with food, shelter, utilities, transportation, personal care products, clothing, and other emergent needs. Connection to other available services and resources.

Senior Buddies Calling Program: Meaningful connections, resource needs assessment and weekly check-in calls for elderly residents living in Franklin County.

Substance Use Prevention: Community resources and education for youth and adults on alcohol, marijuana, opioids and other drugs.

Substance Use Recovery Resources: Community stigma trainings, Peer Recovery Coach program, community resources and education and Narcan education and distribution.

Tobacco Prevention and Recovery: Assistance with tobacco-free policy development and modification. Provides youth education and prevention. Support for individuals weighing to quit nicotine.

Vaccine Access and Information: Information and access to COVID-19 vaccines and flu shots.

Youth Engagement: Community support and advocacy for youth and adult mentors.
